Dental Insurance

Effective 1/1/2009 the USG Indemnity Dental plan provider will be MetLife. The benefit structure and monthly premiums will remain unchanged. Dental Coverage provides for two cleanings and check-ups per year. Other covered expenses are paid at 80% (after satisfying a $50 per person annual deductible) up to a $1,000 annual maximum per person.


Dental insurance must be elected at the time of hire, or during the 2008 Open Enrollment period. There is no plan for dental insurance to be available during later open enrollment periods.
