Hepatitis B Declination (if needed) ___3. Current CPR          (copy of card)  American Heart Association – Health Care Provider           Two man rescue includes adults, children & infants. ___4. Photo (printed snapshot is fine) ___5. Accident/Illness Waiver Form (Signed and Notarized) ___6. Liability Insurance Form ___7. Name Tag Form ___8.
